30-40 mmHg extra firm compression is generally reserved for more advanced conditions — significant chronic venous insufficiency, lymphedema-related swelling severe enough to need ongoing management, or post-thrombotic syndrome (chronic leg symptoms that can develop after a prior blood clot, or DVT). It's almost always worn by people actively managing a diagnosed condition long-term, rather than as a starting point — most people wearing this level have been through lighter compression first and have a provider actively involved in their ongoing care plan. Because of how strong this compression is, it's typically prescribed or recommended as part of an ongoing care plan with a provider, rather than something most people choose on their own without that guidance already in place.

30-40 mmHg is best used when a healthcare provider has specifically recommended this strength for your condition. If you haven't been advised to use extra firm compression, starting at a lighter level — or speaking with a provider first — is the safer path, since a precise fit matters more here and an incorrect one can be uncomfortable or less effective than intended.

This is the firmest level we carry, so there's nowhere higher to compare to. Women's 20-30 mmHg firm compression suits moderate to severe symptoms that don't require extra-firm support. Women's 15-20 mmHg moderate compression fits milder symptoms and standing occupations. Women's 8-15 mmHg light compression is best for gentle daily support when no specific condition has been diagnosed.

At this firmness, a donning glove or stocking donner moves from helpful to close to essential for most people — this is the level where the same firmness that makes the compression effective also makes it hardest to apply by hand alone. Plan to replace them roughly every 3 months with daily wear.

Because 30-40 mmHg is typically part of a provider-directed treatment plan, it may have a stronger case for insurance or FSA/HSA reimbursement than lighter levels, but coverage still depends on your plan and documentation requirements.
